That "they know you will file a claim" is simply not absolute. They don't know. I do see that you made a reference to healthcare. Perhaps in that specific industry, you may be right. But insurance as a whole? Not entirely. And homeowners insurance (which is what kicked off the topic) ... I don't think I have ever filed a claim with my homeowners insurance all my life.
